Altitude Physiology & Clinical Considerations
When a patient travels by aircraft, cabin altitude typically fluctuates between 5,000 and 8,000 feet in pressurized jets, or higher in unpressurized aircraft. This drop in barometric pressure causes gases inside the body to expand (Boyle's Law) and decreases the partial pressure of oxygen in arterial blood (Dalton's Law).
Conditions Requiring Specialized High-Altitude Management:
- Pneumothorax: Trapped air in the pleural cavity must be drained with a chest tube prior to flight to prevent tension pneumothorax.
- Recent Eye or Intracranial Surgery: Trapped intraocular or intracranial air requires sea-level cabin pressurization protocols.
- Severe Anemia (Hb < 7.0 g/dL): Oxygen-carrying capacity is reduced, requiring supplemental high-flow FiO2 or pre-flight transfusion.
- Severe Bowel Obstruction: Intestinal gas expansion necessitates nasogastric tube decompression during flight.
